A Basic Assessment of Hildegard Peplau's Theory Essay

The role of the nurse in the healthcare setting is to be supportive, informative, advocate and help patient through their treatment journey through the use of language- verbal and non-verbal.

Peplau defines nursing as the therapeutic interpersonal, serial, goal- oriented process. A health-focused human relationship( Meleis, p 165, 2012)

The nurse also has to be aware of the nature of the nurse-patient relationship, the environment in which it takes place in, as well as an understanding of the context and the moment of the interactions in order to share a meaningful experience. Finally, nurses have to be passionate about his or her career. Nurse’s care and tend to the patients’ needs because nurses are passionate about the career. Even though the fundamental pattern of personal knowing, it provides a holistic nature of knowing. Many humanistic theories and models of nursing have been derived from personal knowing such as Peplau’s interpersonal relation theory, Kolcaba’s comfort theory, and Jean Watson’s theory of caring.

“Nursing encompasses autonomous and collaborative care of individuals of all ages, families, groups and communities, sick or well and in all settings. Nursing includes the promotion of health, prevention of illness, and the care of ill, disabled and dying people. Advocacy, promotion of a safe environment, research, participation in shaping health policy and in patient and health systems management, and education are also key nursing roles” (ICN 2010)

The nurse’s role would be to evaluate first the patient’s level of understanding of their disease. It is important to also include a patient’s support system when teaching. The nurse must give the patient information about his or her condition in a form he or she can easily understand so the patient can develop a genuine comprehension of the disease process. After the patient has a good comprehension of the disease, he or she can then apply this knowledge to his or her own situation. When this knowledge is applied to a patient’s own situation, the patient is then able to ask questions and form a new way of thinking about living with the disease. When these steps are accomplished, the patient will then be able to create a plan that would help in achieving his or her goals in regards to living with the disease process. As nurses caring for patients with chronic illnesses, recognizing where patients are at in their journey with learning about their chronic illness, and meeting them where they are will help when educating and guiding them to the next step.

My personal definition of nursing is taking care of my patient as a whole; using my knowledge, being compassionate and caring, respectful and honest. Taking care of the; spiritual, physical and emotional aspect of the patient, and taking into consideration their family and environment. Learning about the different theorists I associate myself the most with Hildegard Peplau. Peplau believed that the relationship between the patient and the nurse was focus of attention, rather than the patient only as the unit of attention. (Chitty & Black p.314-315). Using Peplau’s theory my relationship begins with my patients as strangers and I try to

Hildegard Peplau’s middle-range theory, Interpersonal Relations, established in 1948 and highlighted the nurse-patient relationship as the groundwork of nursing practice. Peplau supports each role individually, the nurse and the patient, by ensuring that equal participation is implemented to reach a mutual goal. For example, the role of the patient reflects vulnerability and the expectation to be open to the health care providers throughout care and treatment plan. Subsequently, the nurse is supposed to facilitate a reciprocated relationship by noting the client’s behavior, defining the client’s illness, creating the most proper care for the client, understanding the physician’s treatment plan and confirming it, and intervening when required (Sue Penckofer, 2013).

With many theories on nursing, the nurse must determine which approach is best suited to her particular skills and patient population. In 1952 Hildegard E. Peplau presented her theory on interpersonal relations in regard to nursing care and practice, providing a model for nurses to aid patients in healing and overcoming illness through personal interaction (Cherry & Jacob, 2011, p. 96).

Its usage is to manage communication between an organization and a public while maintaining boundaries in the therapeutic relationship. Based on Peplau’s interpersonal theory, communication takes place in a nurse-client relationship where therapeutic process occurs involving complex factors such as environment, attitudes, practices, and beliefs in the dominant culture (seu.edu, 2015). The actions of each person in a nurse-patient relationship is measured on the collaboration of their thoughts, feelings, and experiences. Nurse’s work to attain, maintain, and restore the patient’s health until patient have fulfilled the health care needs. Patient must be guided and provided a well-respected environment until a better health and specific needs are fully considered in the relationship. In this kind of setting, nurse’s must create relationship with patients by communicating receptivity, assimilating the concepts of empathy, trust, genuineness, respect, and confidentiality into their interactions.

Nurses support and enable individuals, families and groups to maintain, restore or improve their health status. Nurse also care for and comfort when deterioration of health has become irreversible. A traditional ideal of nursing is caring and nurturing of human beings regardless of race, religion, status, age, gender, diagnosis, or any other grounds.

Nurses are known as the heart of health care. Being a nurse is a demanding job that requires commitment, but does not lack rewards. A fact stated by the American Association of Colleges of Nurses claims that “Nurses comprise the largest single component of hospital staff, are the primary providers of hospital patient care, and deliver most of the nation 's long-term care” (“Nursing Fact Sheet”). With many roles throughout the healthcare system as a whole, nurses are a large, very important role that interacts with every other part of the health care system. They have great qualities that not everyone has. They are highly compassionate, caring, professional, diligent and understanding individuals. A nurse experiences people at their worst and still care for patients in a way that no one else would. All these honest points prove that nurses are the most important members of the health care system because, nurses spend the most time with patients, nurses are the managers of patient care; they are teachers and are great with conflict resolution.

Nursing theory is the backbone of nursing. It is very important in the application of evidence-based practice (EBP). The functions of a theory are to narrow and provide specific information for the analysis of initially confusing behaviors, situations, and events (Fawcett & Madeya 2013). Moreover, nursing theory helps to improve not only the quality of care that we are providing to our patients but, they also help us make better clinical decisions. As a matter of fact, nursing theory gives the nurse a framework for organizing and analyzing information as well as a base for identifying with their patients throughout the nurse-patient relationship (Schick, 2015). I am very much interested in all the theories. The one that I relate to the most is Hildegard Peplau’s theory. It is a middle range theory that focuses on interpersonal relations. This theory teaches us about how to interact with our patients so that they feel more in control of their treatment. In this paper, I will be talking about the importance of nursing, the summary of Peplau's theory, and how as future nurse practitioners, we can apply it.
