Question 3
Priority Problems
The two priority problems for Mr. George are Hypertension and Diabetes (hyperglycemia). It has been demonstrated that arterial hypertension is major important determinant of heart diseases. The pathogenesis of hypertension is linked with multiple factors that include dietary, genetic, and patterns of lifestyle. The biological factors that determine hypertension include: atherosclerosis, resistance to insulin; hyper-responsiveness towards adrenalin based stimuli and dysfunction of endothelial cells. It was studied that the evidence suggests that decrease in the ratio of prevalence of high blood pressure gives rise to reduced ratio of cardiovascular related mortalities. It can be achieved through intervention with suitable medical therapy (Villalpando et al., 2010).

High blood pressure is also referred to ‘silent killer”. Hypertension leads to stroke, myocardial infarction, renal failure, and ultimately death, if the issues are not diagnosed early and well- treated (James et al., 2013). The association between high blood pressure and alcohol consumption was also studied according to which measurement of patterns of time, were recorded with reference to elevations in blood pressure due to alcohol consumption. It was found that a considerable increase in blood pressure at the time of morning; was associated with alcohol consumption independently of smoking by means of a dose-dependent pattern. There were twelve fold increments in risk for heart diseases in Men with severe hypertension. In addition, hypertensive patients, if drink alcohol then risk increases further and leads towards stroke (Hillbom, Saloheimo & Juvela,

Alcohol and hypertension are two rising concerns in national and global health. According to the Substance Abuse and Mental Health Services Administration (2014), 56.9% of adults in the United States have participated in drinking in the last month. Furthermore, 24.7% adults reported to participating in heavy or binge drinking. The World Health Organization (WHO) reports that in 2012, 3.3 million deaths or 5.9% of all global deaths were linked to alcohol consumption and within the United States 88,000 people died from alcohol related causes making it the fourth leading preventable cause of death in the United States (Stahre, Roeber, & Kanny, 2014). However, binge drinking or drinking too much over an extended period of time can damage the heart and cause problems with the heart like strokes, high blood pressure, abnormal heart beat, and alcoholic cardiomyopathy. When blood is not able to reach the brain it causes a stroke. Majority of strokes occur when blood cannot flow to the brain because of a blood clot which is also called an ischemic stroke. Strokes can be caused when there is binge drinking or long term heavy drinking even when people do not have coronary heart disease. Alcoholic Cardiomyopathy, which is when the the heart muscles are weakened, is caused by long term excessive drinking. When a heart is weak it tends to stretch and makes contracting not as effective and leads to not enough blood being pumped to the organs which leads to organ failure. Long term use of alcohol as well as binge drinking can lead to high blood pressure also called hypertension. Normal blood vessels are elastic and can stretch but hypertension is when the vessels stiffen making them less elastic. Copious alcohol consumption can raise blood pressure and may affect the use of the muscles within the heart and blood vessels. The human heart relies on an internal pacemaker system to keep it beating at the right speed. When a human drinks alcohol it can disturb the internal pacemaker and may cause it to beat
Throughout this paper, I will state the cardiovascular system’s role in the body and define the elements that compose blood pressure. As well as emphasize the risk factors, pathophysiology, and management of hypertension.
Cardiovascular disease is the leading cause of mortality and morbidity in adults worldwide and it accounts for approximately one-third of mortality in Canada and in the United States. Elevated blood pressure (BP) is also another major cause of death worldwide. The reduction of BP is a cornerstone of the prevention of cardiovascular disease (CVD), there are numerous hypertensive patients that do not achieve adequate blood pressure control. In the United States in 2009-2010, there was an estimation of 53% of all hypertensive people and 40% of treated hypertensive people had uncontrolled BP. The lower rates of blood pressure control have been reported in European countries.
