Dodge Durango Incident Summary

Upon arrival, Officer Wheeler activated his (BWC1) body worn camera which was later uploaded to evidence.com. As our patrol vehicle pulled up to St. Luke’s emergency room entrance, I viewed a male later identified as (RV1) Devinelle Lavaughn Broussard with a cellphone in his hand attempting to prevent a female, later identified as (B1) Eleasia Ashanti Fraise, from throwing a (EVD1) red brick. I next saw Fraise throw the brick at a parked (D1) silver BMW, located along the south side walk. The brick fell to the ground and Fraise went to picked it back up. Fraise picked the brick up and moved in the direction of our patrol vehicle with the brick at shoulder height. I parked the patrol vehicle and immediately got out drawing my firearm to a low ready. Officer Broussard stepped out of the vehicle and drew firearm pointing it at Fraise (View Officer Wheeler’s statement for further details). I viewed video surveillance from LRHS camera #7.24 at approximately 3:04 p.m. and observed a Red pick-up while backing up strike the vehicle in question left rear back fender near the back tire. At approximately 3:05 p.m I observed the suspect getting out of the truck and walking up to the vehicle in question and kneeling down near the left rear area as if he was assessing the damage. At approximately 1:28 a.m., I arrived at the scene and met with Trooper Otten, who advised what had transpired prior to my arrival. Trooper Otten stated a blue 2014 Ford F-150, driven by Eric A. Wenzel, was traveling northbound on Illinois Route 4 south of Norton Lane. While traveling northbound, the blue Ford veered to the left and entered the southbound lane of the roadway. Once in the southbound lane, the front passenger’s side of the blue Ford struck the front passenger’s side of a southbound silver 2003 Ford F-150, driven by Alan L. Schilling. Trooper Otten stated both Mr. Wenzel and Mr. Schilling sustained incapacitating injuries in the collision Trooper Otten advised the silver Ford’s front seat passenger, Sarah L. Wagner-Schilling, sustained life-threatening injuries in the collision and was not expected to survive. Trooper Otten said Mr. Wenzel admitted to having been drinking prior to the collision and was suspected of driving under the influence. All three occupants had been transported from the scene

In March 2001, a Georgia deputy attempted to pull over 19-year-old Victor Harris for driving 73 miles-per-hour in a 55 miles-per-hour zone. Instead of slowing down, Harris increased his speed, which initiated a chase at speeds exceeding 85 miles-per-hour. The deputy radioed for backup to pursue him. Deputy Timothy Scott, the petitioner in this case, joined the group of officers pursuing Harris. The officers attempted to box Harris into a shopping center parking lot, but he made a sharp turn, collided with Deputy Scott, and sped off again. Deputy Scott took over as the lead pursuit officer in the chase.

On Thursday, July 14, 2016, at approximately 0847 hours, Patrolman David Stamets had a vehicle stopped on the 100 block of East Oak Street. Immediately after Stamets stopped the vehicle Your Affiant positioned his vehicle on the 100 block of South White Street near the Oak Street intersection facing southbound. When exiting my vehicle your Affiant observed a gold Pontiac sedan accelerating northbound on South White Street at an unsafe speed towards Stamets and your Affiant's location.

At approximately 1:13 p.m., I arrived at the scene and met with Trooper Cox. Trooper Cox advised me what had transpired prior to my arrival. Trooper Cox said a gold 2000 Saturn LS1 and a black 2011 Ford Mustang GT were traveling the wrong way

On August 23, 2014 Sgt. Lavelle was assigned to the 83 Precinct. She was present for duty at 0800 hours as the domestic violence Sergeant and she conducted several home visits. At 1450 hours she was post changed to the 83rd Precinct desk and she assumed the duties of the 83 desk office. Sgt. Lavelle remained as the Desk Sergeant for the entire tour. I presented Sergeant Lavelle with a copy of the command log entry for August 23, 2014 depicting the validation of two arrests at about 2040 hours when Mr. Carlton Smith

In summary, on 01/04/17 at 1745 hours, Ofc. Kelley #262 and I were patrolling the area of 5600 W 16th St., at which time I observed a vehicle with tinted windows and no rear registration light (05' grey Acura IL Q336438). A traffic stop was conducted on the vehicle.

On Sunday November 24, 2016 at approximately 16:02 hrs. Officer L Wilkerson and I Ofc. R. Holcomb were dispatched by dispatcher B Coleman to 304 Hopkins Drive in reference to a disturbance. Upon my arrival I made contact with Ms. Shawntavia Lucky (complainant) who stated that her son Mr. Jousha Lucky (victim) got into a physical altercation with two black males.

Unit 2 was driving eastbound in the left lane of 167th St. between Orchard Ridge Ave. and Anthony Ave. in Hazel Crest, IL. Unit 2's approximate speed was 35 miles per hour. A large puddle of water had formed from a very recent rain. It covered the right lane and part of the left lane. The driver of Unit 2 noticed the puddle and proceeded to slow down to a speed of approximately 20 miles per hour. The driver of Unit 2 then noticed Unit 1 coming from behind in the right lane at a high rate of speed. Unit 1 tried to avoid the puddle without slowing down and impacted Unit 2. Unit 1 was pushed back into the right lane. Unit 2 was pushed off of the left side of the road onto the slightly raised median. The front and left side of Unit 1 was severly

Deputy Martin said he was traveling approximately 60 mph and stated there was no visible traffic. Deputy Martin followed the vehicle for approximately another .3 mile(s) before entering the private drive of 1500 Wooten Road. On 10/28/2015, at approximately 0232 hours, Captain O'Neill and your Affiant observed a red Chrysler PT Cruiser (PA Registration JWA5451) traveling south on State Route 924. The operator of the vehicle was observed and identified as Dawn HERRING. HERRING'S driver's license was expired and suspended for driving under the influence; the information was verified through a certified Pennsylvania Department of Transportation record. HERRING was observed traveling under the posted speed limit and repeatedly swayed her vehicle throughout the lane. Your Affiants activated the emergency lights on the marked patrol vehicle and stopped HERRING at the entrance of the Transfer Station.

On March 3rd 1991, Los Angeles Police chase a vehicle that was driving erratically. When the vehicle came to a stop after reaching speeds of nearly 120 mph, a total of four cruisers arrived to the scene, including six patrol officers and a sergeant. ON 09-03-2016 at 0930 hours, I was notified by Sgt Kelley of an aggravated battery which occurred at 2111 Roanoke Springs Drive. I was advised to respond to St. Joseph’s Hospital (South) at 6901 Simmons Loop in Riverview. Upon my arrival, I met with Sgt. Kelley, Deputy Karpenske # 249578 and Crime Scene Tech Dewitt. There was a black 2013 Nissan Maxima bearing Florida tag, DGV F36, parked in the emergency room parking bay. I learned the suspect, Joanna Lebrea Lewis had driven the car to the hospital for treatment of the injuries sustained during the offense. It was learned that verbal consent had been obtained from both the victim and suspect to search their residence and vehicle. It was also learned the victim, Marieio Denod Spradley,
