Payment was denied because Medicare does not cover routine and related services, such as the vascular study 93978-26. There was no indication of symptoms or physical findings that would support the medical necessity of the procedure performed. The performance of the service must be clearly documented in the patient’s medical record in order to establish the medical necessity. Medicare guidelines state that services are excluded from coverage when performed for a purpose other than treatment of a specific illness, symptom, complaint, or injury.
