“Bipolar disorder, also commonly known as manic depression, is defined as a serious mental illness in which common emotions become intensely and often unpredictably magnified. Individuals with bipolar disorder can quickly alternate from extremes of happiness, energy and clarity to sadness, fatigue and confusion. All people with bipolar disorder have manic episodes abnormally elevated or irritable moods that last at least a week and impair functioning. But not all become depressed ” (American Psychological Association, 2015). Bipolar disorder can vary in each individual. The symptoms fluctuate in pattern, severity and rate of recurrence. Some people are more susceptible to either mania or depression, while others change proportionately between the two types of episodes. Some have frequent mood disruptions, while others live through a few throughout their lifetime.

Bipolar Disorder is a brain disorder that is characterized by abnormal changes in mood, energy and activity levels (“National Institute of Mental Health”). Manic-depressive illness is also another name that bipolar disorder is commonly referred to as (“National Institute of Mental Health”). This disorder has four basic types including, bipolar I, bipolar II, cyclothymic, and any other which do not perfectly fit the first three. All four of the types have episodes of extreme highs, manic periods, and extreme lows, depressive periods. Symptoms can range greatly and depend on what type of episode the affected is having. There are different symptoms and different types of bipolar disorder. The primary symptoms of bipolar disorder are dramatic and unpredictable mood swings. Mania symptoms may include excessive happiness, excitement, restlessness, less need for sleep, and high sex drive. Depression symptoms include sadness, anxiety, loss of energy, uncontrollable crying, need for sleep, and thoughts of death or suicide.

The mood disorder of bipolar is a roller coaster of high and low emotions. Bipolar has many different components and can manifest as either Bipolar I or Bipolar ( Oltmanns & Emery, 2015) Bipolar I is described as having one manic episode. ( Oltmanns & Emery ,2015) Mania is a disturbance in mood characterized by symptoms as elation, higher self-esteem, hyperactivity and expedited thought process. (Oltmanns & Emery, 2015) To have Bipolar II, a person must have at least one depressive episode and a mild manic episode. A depressive episode includes hypomania meaning in bipolar II a person will not have a full blown manic episode. (Oltmanns & Emery, 2015) People who have bipolar have a tough time with their emotions, because their emotions are not stable. They oftentimes feel like they are on an emotional roller-coaster because of the erratic ups and downs ( Oltmanns & Emery 2015)

While having a manic phase people with bipolar disorder cannot control themselves, their behaviors, they do not feel sleepy or tired, they are very involved in activities, and they can be very hyper , they can have very elevated mood.

Mentors should reflect confidence, hope, optimism, and resiliency as well as the ability to promote others and their strengths. This sounds uncannily like the descriptions of an authentic leader that we have been reading about in our literature.

1. What is the nurse's highest priority for a patient experiencing sleep deprivation? What would we teach them about this? What treatment would be expected? Safety would be highest priority; sleep deprivation causes psychomotor deficits. Interventions that can help with sleep deprivation would be avoiding stimulating activities in the hours before bed. Avoid exercise, caffeine, and screen time just before bed. It’s also a good idea to avoid eating a large meal, as this can interfere with healthy sleep. Create a comfortable sleep environment. One medication that the patient can be prescribed on would be Zolpidem is a short-acting hypnotic that will help the patient initiate sleep and awaken without untoward symptoms of drowsiness. Also, Gamma-aminobutyric acid (GABA) and galanin are sleep-promoting neurotransmitters that can be used as a treatment.

As I stated earlier, bipolar disorder consists of shifts in mood, energy, and activity levels. This can mean someone can seem very sad and depressed for a long period of time and then a period of extreme happiness. These can be shown in mood or behavioral changes and can be shown in how well they are able to concentrate, how much sleep they are getting, or even if they have thoughts of suicide (NIMH). Although the easiest way to spot bipolar is through extreme mood swings, it can still be present when such mood swings are less noticeable. It is important to look out for even the smallest mood swings because if they are frequent enough it can be a sign of one having bipolar disorder.

The patient Maria Soto presents as an able bodied 35 year old Hispanic female. She is currently unemployed and residing with her boyfriend in East Los Angeles. She falls within the low income bracket. The patient is seeking therapy for depression. In the textbook Essentials of Understanding Abnormal Behavior defines depression as a “mood state characterized by intense sadness or despair, feelings of worthlessness, and withdrawal from others.” (Sue, Wing Sue & Sue, 2005). I will also be using the Fundamental of Abnormal Psychology by Ronald J Comer to support the diagnosis and provide treatment plan for Maria Soto.

The main symptom of bipolar disorder would be the dramatic and irregular mood swings. These mood swings would consist of the person being in a great mood one second, and then being extremely angry the next.
