In the beginning of this activity, I did not know much about collaboration between different healthcare professionals. It helped me to clarify the meaning of a healthcare team and also to understand the role of different professionals in the team. Different team members have their specific roles and all of them work together to achieve a common goal –healthier patient. They work independently, but when it comes to decision making they seek advice from other healthcare provider in order to do the best for the patient. As time goes by, people get infected with new diseases or current prevailing diseases. Consequently, new drugs are produced in an attempt to overcome these diseases, which results in patients with complex health needs. The complexity of the patients’ healthcare therefore needs to be addressed by more than one discipline. Interprofessional collaborative care is a type of health care that involves people from different professions working together and relying on each other to provide effective care to patients. Interprofessional collaborative care is important and predominantly a focus of the health care reform because it improves healthcare outcome for the patients and reduces disagreements between different professions. I was able to experience interprofessional care when my grandma was diagnosed with cancer. The physician and the pharmacist worked together in ensuring that she got the proper medication that would not have any side effects on her. In addition, the physician would call the pharmacy every two weeks to check on my grandma’s progress and to ensure that she had picked up her medications. Both my grandma and I were grateful because we felt that our needs were taken care of appropriately. Interprofessional team collaboration for professional nurses is viewed as a method to improve the care and safety for patients. However, interprofessional team collaboration presents both advantages and challenges for nurses and other team members. One of the advantages is the coordination of care for the patient and the sharing of knowledge to improve the outcomes for the patient. Challenges for interprofessional team collaboration is: poor role-definition, miscommunication, conflict, lack of accountability for assignment of responsibilities and tasks (Reeves, 2012). Interprofessional Practice (IPP) occurs when different professional groups work together and rely on one another to accomplish common goals and improve healthcare outcomes. The way in which healthcare professionals interact and communicate with one another affects the quality of healthcare they provide and can effect patient care and safety (Australasian Interprofessional Practice and Education Network). Barriers to effective interprofessional practice between doctors and nurses have been well documented and include problem power dynamics, poor communication, lack of understanding of one another’s roles and responsibilities and from conflict arising due to different approaches to patient care (Zwarenstein, Goldman & Reeves, 2009)

In healthcare settings a multidisciplinary team (MDT) approach is required to provide a safe and quality holistic care for patients. To achieve this, the different healthcare professionals must work together in order to achieve their goals and promote patient safety. This assignment aims to explore the importance of team work in healthcare. In addition it will consider the qualities and behaviours of a team leader as well as the team behaviours necessary for effective team performance.

The final course project was creating an interprofessional team (IPT), function as a leader and take part in the stages of group development, setting of the team roles and goals, identifying communication patterns, and team analysis to discuss an issue at the microsystem level at our facility. The issue that is being addressed is post-transplant steroid associated hyperglycemia. For patients that have new onset diabetes after transplant (NODAT) whether immediately post-operative period or during a rejection episode is an issue that needs attention. The patient has a poor understanding of the blood sugar monitoring, insulin administering, and dieting. A date, time, and location was secured that was conducive to the team members schedule.

In my NUR-301 Class, I had the opportunity to practice on Interprofessional Education. Our nursing student team was partnered with first year pharmacy school students to learn about each other professions and to study on how we could work in a team of healthcare professionals to attain a common goal. As this was my first time experience to meet other students from a different department of Notre Dame of Maryland University, I was not sure what to expect from our discussions. Even if I have some experiences working both in hospitals and in a pharmacy, I was no fully aware of the responsibilities of a pharmacist directly to the patient or to the healthcare field specialists.

Working in a team is an important responsibility by understanding each other’s role which may include doctor, nurse, occupational therapist, physiotherapist and many more. Team members divide the work based on their scope practice such as acute care, metal health care, homecare etc. Interprofessional collaboration practice is decision making and communicating between individuals for their patient’s health based on their knowledge and skills. It helps to promote habits, maximizing health resources, leading care to be safer with patient’s satisfaction and Canada’s health care (Kenaszchuk, Reeves, Nicholas, & Zwarenstein, 2010).

Over the course of Trimester A, I had the opportunity to be a part of an interesting module known as “Interprofessional Context of Practice”. This module continually challenges the biomedical approach of western medicine by examining the collaborative practice of various professions (Robertson, Kinrade, and Green, 2015). The knowledge and understanding of the interprofessional realms of health care was assessed through group presentations. It was during this assessment where I have gained the most amount of interprofessional experience. As such, I will reflect upon an event that occurred during the presentation process.

In order to collaborate successfully with the other members of the team, they have to ‘work together’. Collaboration implies “working together to achieve something that neither agency could achieve alone” (cited by Biggs in Day, 2006, p9). It involves effective communication and contribution to a common goal – and the health and wellbeing of the patient and shared responsibility of the outcome. Each team requires a quality leader, regular meetings attended by all members, joint assessment, regular reviews of patient records which should include ‘shared care plans’; joint decisions following consultation and task delegation to individual team members with the outcome being that “care must be structured, organised and systematically provided to each person in a variety of ways” (Creating an Interprofessional Workforce, 2007, p10).

In this essay I will concentrate on inter-professional collaboration which is cornerstone for providing better care to patients. I will try to examine some benefits and opportunities, while identifying the barriers and issues to effective collaborative work between employees. Throughout my essay I will be using LEARN format for better reflection on my clinical practice, my thoughts and feeling. As a nursing student in Early Identification team which is focused on maternal and children’s health, I will discuss my experience as being a part of this team.
