I sent an email to the Director Mr. Robert and the supervisor Mr. Ashwin about the results that I have found after analyzing data. I have found interesting results. I found there are a statistically significant variation in the number of ER visits and inpatient hospitalizations for Zip Codes 65616. This is for individuals who are lacking the mental capacity and need somebody else to help them manage their legal, financial and health problems. The mental capacity act therefore made it legal so that those who are unable to make their own decisions have the ability to choose somebody who they feel they can trust to help manage their finances, properties and to help make their health and welfare decision. This is done through the power of attorney. This act links to individuals who suffer with dementia as within the late stages of dementia they will need to give someone the power of attorney due to the fact that they will not be able to manage their legal financial and health problems.
Over the year both positive and negative influences have influenced my learning. The skill I have developed for learning is my aspiration is to become a midwife, this influences my learning in the way that it gives me the hunger to learn new things and improve my knowledge and grades. By improving my knowledge I will be able to go onto the next stage of my development. I believe that if I did not have such a desire to be a midwife, it would affect my learning by creating a mental blockage which would stop my ability to take things in and learn as I would become lazy and not interested from the lack of direction in my life. Health Promotion This is the process of allowing people to have more control over their health as this will help improve their health and well-being. It focuses on individual behaviour towards a wide range of social and interventions. Health promotion is how health is shown for example to challenge obesity change 4 life is used so people can have a guidance on how to make their lifestyle healthier. It also shows that health promotion is aimed at individuals whose health is at risk from any diseases from lifestyle factors, which lets individuals take control over their health. By being able to have control over their health it shows individuals what may have happened if nothing was done of it
In this report I will be describing different patterns of ill health and who and how they are monitored. I will also try to explain different factors that affect our health, not only now but I will try to discuss how different factors fight effect our health in the future, I will then evaluate the influence of the government have on factors that affect ill health.
A health service is a service that provides care and is under the NHS. For examples hospitals or a G.P. They provide treatments but also prevent illnesses through health promotions. A social care service is about providing support to individuals to help them get by and who ever needs it such as emotional support or practical support with daily living tasks.

There are seven types of hazards that exist in the health and social care setting and one of them is the Physical Environment. The physical environment consists of : high or low ‘temperature’ in rooms, objects which may put others in danger, individuals who cause physical harm to patients and staff, poor ventilation which can affect the air.
Although, towards the end (July/August) it became dismantled as a result of other projects Yale New Haven residency program adapted. However, open communication remains between the residents and Pediatric Chiefs and the program coordinator. The collaboration provided the pediatric residents and interns’ opportunity to acquire knowledge about Community Public Health Programs. With the primary goal of introducing the residents and interns to a broad range of public health programs they can refer their patients to. This system offered the residents, and interns an introduction to the connection of public health and medicine. With the end result demonstrating the importance of community agencies and medical providers need to partner to address the burden of asthma. The Region had 5 residents/interns come through this reporting period, The residents have reported that given this opportunity to see first-hand how the program works, whether within the home or through individual discussion, that it has helped in aiding to understand the challenges families face as they strive to manage the asthmatic’s symptoms. My recommendation for TCHD is TCHD has a partnership with ED in order to educate asthmatic patients about how to control asthma in a proper way.
